Dear Rare Patient Warriors and Friends,

Patient Worthy is having a personal essay contest for a rare patient, caregiver or support person who hopes to attend the Set Sail for CMT Event on June 17th  in New York City.  The winner of the essay contest will receive two tickets to the event (or have the cost covered if they’ve already purchased the tickets). All contestants have a chance to be featured on Patient Worthy’s site!

 To Enter:

  • Submit 400-800 words on a rare disease experience you have had, including experiences with treatments or therapies that have helped symptoms of CMT or your rare condition.
  • If you are a caregiver, support-person or physician, same prompt, just from your unique perspective.
  • E-mail a word document of your essay to [email protected] by Friday, 9 am, June 9, 2017.

Upon receiving all submissions, a winner will be chosen and will either be reimbursed for the cost of two tickets, or receive two tickets to the event. If you are chosen to win two tickets, we will contact you and thHereditary Neuropathy Foundation. If you are chosen to be published, we will follow up before publishing your submission on the Patient Worthy site.

Please review the terms of our Contributor Agreement by clicking here. By entering into the contest, you agree to these terms. If you have any photos, images, videos, or other media files that you want to be included with your submission, simply attach them to your email.
